Complaint
- Onset fever for the past 3-4 months, fever would reach 102-104°F.
- Frequent hospital admission, each for at least 15 days.
- Temperature remained 99-100°F after discharge from hospital.
- Drastic reduction in weight.
While narrating their story, they mentioned visiting a crowded place at the hilltop and they thought, perhaps their son got exposed to some infection from somebody there.
Elaborate investigations were carried out in the hospital, including Widal, malarial parasite, dengue, blood/ urine Culture, Tuberculin test, and tumor marker tests apart from immunoglobin profile. Except for iron deficient anemia, all the tests revealed a normal pattern.
Integrated Summary
After a thorough examination, advised the patient to consume 5 ml of cow’s ghee, empty stomach in the morning apart from an energy drink (Tarpak yog with water as medium) comprising of black raisins, dates, pomegranates and goose berries. Looking at his good appetite, he was also asked to eat a full meal diet (Shadrasa Sampoorna Ahaar) in banana leaf for at least 7 days. We stopped all his current medications as they were not helping him in bringing down the temperature.
It was observed that he responded immediately to this pattern of diet, where his afebrile condition was seen after almost 4 months. Since he was enjoying his feast, we could see some positive change in his bulk weight (Mansa-upachaya).
Treatment duration: 21 days
Etiology: Climbing of the hill.
Signs & Symptoms: As mentioned above.

| Treatment | Dosage |

Conclusion
His fever was considered as Vattaja Jwara since the cause was from climbing of hill and not the exposure to crowd. Cow’s ghee served to pacify the chronicity of fever-Jeernatwa (Ashtanga Hridayam Jwara adhyay), whereas the tarpana drinks along with a full meal in banana leaf pacified Vatta to a great extent. A mere treatment to reduce temperature is not just enough to pacify any febrile condition, it is very important to derive the root cause to pacify the aggravated elements.
